Resumen
Introducción: Las mujeres con lupus eritematoso sistémico (LES) que cursan un embarazo presentan un mayor riesgo de complicaciones maternas y fetales.
Objetivo: Describir las características clínicas y los resultados maternos y fetales en mujeres con LES embarazadas, atendidas en un hospital de referencia en Paraguay, durante el periodo enero de 2019 a abril de 2026.
Materiales y métodos: Estudio descriptivo retrospectivo, que incluyó mujeres con LES y embarazo.
Resultados: Se incluyeron 31 pacientes con edades entre 18 y 40 años (media 27 ± 5,6 años) y una duración media de la enfermedad de 6 ± 2,9 años (rango: 1–15). Dieciséis pacientes 16/31 (51,6%) presentaron actividad de la enfermedad, de las cuales 8 (53,3%) tenían nefritis lúpica. El embarazo no fue planificado en 26/26 casos (100%), y en 5 pacientes (16,1%) el LES debutó durante el embarazo. La complicación obstétrica más frecuente fue la preeclampsia, observada en 9 de los 31 embarazos. Se registró una muerte materna. El parto prematuro ocurrió en 10 casos (32,2%) y se documentaron 6 óbitos fetales. En cuanto a los marcadores inmunológicos, los anticuerpos anti-ADN fueron positivos en 14 pacientes (70%) y los anticuerpos anti-Ro en 17 (77,2%). Una paciente presentaba antecedentes de síndrome antifosfolípido, cuyo embarazo evolucionó sin complicaciones.
Conclusión: La mayoría de los embarazos no fueron planificados. Casi la mitad de las pacientes presentó actividad del LES durante la gestación. La preeclampsia fue la complicación obstétrica más frecuente, con una muerte materna y el 20% de pérdidas fetales.
